Program Directors
Our Program Directors oversees the general welfare of his or her designed home/apartment setting. He or she is in direct contact with Case Managers, guardians, and family members to develop an individualized care plan for each consumer.
Registered Nurses
Provides services to consumers in accordance with the State Nurse Practice Act, and agency policy. Each RN is designated to a group of homes within our company to supervise nursing tasks inside the home/apartment setting. Central Minnesota Senior Care, Inc. provides a 24 hour “On Call” Nursing pager system.
Licensed Practical Nurses
Perform skilled nursing functions in accordance with the federal, state, and local laws. Our LPNs provide care to consumers as delegated by the RN Case Manager or our Director of Nursing. Most settings have a LPN as a Program Coordinator.
Program Coordinators
Provide general supervision of the home/apartment setting by completing all necessary reports, plans, and schedules. They provide management to the CNAs and HHAs designated to the setting. Assure each consumer is being provided a proper active treatment. Assure all staff interactions with consumer are respectful, polite, and positive.
Certified Nursing Assistants & Home Health Aides
Direct care staff in the home/apartment setting. Assist consumers with daily activities and perform personal care activities written by the Registered Nurse.
